     The 3-way screw is used to widen and well and lengthen the arch. The anterior section

    will drive forces forward using on the incisors. Alternatively, we can incorporate three

    individual screws, one to widen and two following the sagittal plane to distalize the

    posterior section. As with any expansion appliance, we recommend maximum retention

    with Adams clasps on the 6’s (if erupted) and ball clasps between the pre-molars. You

    can also incorporate a labial bow to control the anterior incisors and a posterior bite plate

    to disarticulate the arches providing additional anchorage for the upper appliance.

    Anterior Repositioning Splint

    The anterior repositioning splint (also referred to as the anti-retrusion, pull forward, or Farrar splint) is used to recapture an anteriorly displaced disc. A definite click in the joint is present. This is a full-coverage maxillary splint with an acrylic flange that rests lingual to the lower anterior teeth, with buccal and incisal indices of the lower cusp tips. This splint brings the mandible into a protrusive position. The anterior repositioning splint is most effective at night because it keeps the mandible forward during sleep when it would otherwise tend to relax and retrude.

    Bionator I

    The Bionator I is used for the correction of Class II malocclusions with a deep bite. It can serve to stimulate alveolar growth for patients with a deep overbite while gaining space and correcting an open bite in mixed dentition cases. The design incorporates fluted grooves in the posterior section along with an anterior bite plate to allow posterior eruption allowing lower facial height. We place an anterior cap for stability as the lower advances into position with the upper.

    Dawson Splint (Cuspid Risers)

    The centric relations diagnostic splint or Dawson Splint is a modification of the flat occlusal plane splint. This appliance has slight centric stops and an anterior ramp extending off the labial cuspid-to-cuspid region to provide incisal and cuspid guidance.  The centric relations diagnostic splint is a flat-plane, full-contact splint fabricated on either the maxillary or mandibular arch. The maxillary splint provides smooth anterior guidance that is just steep enough to allow immediate posterior disclusion.

    The mandibular splint allows for full contact of the maxillary arch on the occlusal surface of the mandibular splint. The anterior region is flattened from cuspid centric stop to cuspid centric stop, which allows for the immediate lift-off onto the cuspids and a smooth transition on the incisors during crossover excursions.

    E-Arch

    Taken from “The Mandibular E-Arch” by Neal Kravitz, the E-arch is a fixed, spring-loaded expander for slow expansion of the maxillary or mandibular arches. Developed by Arnold, and popularized by Berkowitz, the E-arch was invented for maxillary orthopedic expansion in patients with cleft palates. In the mandible, the E-arch can create a modest amount (4 to 5 mm) of arch space in the lower anterior region by orthodontic tipping (rather than orthopedic change) of the lower posterior teeth, as well as distalization of the first molars.

    The appliance consists of a 0.040-inch tube-like frame with a NiTi open coil spring. Seating the E-arch compresses the open coil spring and activates expansion. Therefore, the E-arch is ideal where patient compliance or parent cooperation is a concern. The E- arch is typically banded to the first permanent molars; however, if these teeth have not fully erupted, bands can be placed on the second deciduous molars. In these situations, the clinician may consider placing lingual extension arms to prevent the second deciduous molars from expanding further than the first permanent molars.
